Although we call it a vitamin, vitamin D functions like a hormone in many tissues throughout the body. Nearly every cell has vitamin D receptors, and healthy vitamin D levels support bone health, muscle function, immune health, and many of the systems that become increasingly important during perimenopause and menopause.

Yet vitamin D deficiency is remarkably common in midlife. Some studies estimate that 50% to 80% of menopausal women have low vitamin D levels, which is why checking your vitamin D status is often part of a perimenopause or menopause evaluation.

The challenge is that low vitamin D often doesn't announce itself. Many women have no symptoms at all. Others experience fatigue, muscle weakness, or bone discomfort that can easily be mistaken for the hormonal changes of perimenopause, poor sleep, thyroid disease, or simply getting older. A blood test is the only way to know for sure.

Knowing your vitamin D status isn't about chasing another wellness trend. It's about understanding what your body needs to support strong bones, healthy muscles, and long-term health. Vitamin D matters more in midlife because hormonal changes don’t just affect your cycle; they impact nearly every system in the body. Vitamin D may support several of these systems in important, evidence-backed ways:

  • Bone Health: Bone loss accelerates after menopause as estrogen levels decline. Vitamin D helps your body absorb the calcium needed to build and maintain strong bones. 

    Research from the Study of Women's Health Across the Nation (SWAN) found that women lose about 2% of bone each year during the first four to five years after their final menstrual period, adding up to about 10% to 15% bone loss during the first decade after menopause. Maintaining healthy vitamin D levels is one important part of supporting bone health during this time.

  • Muscle Strength & Balance: Vitamin D does more than support healthy muscles. It also helps your muscles and nervous system work together. This is critical for the balance, coordination, and everyday movement that’s critical for healthy aging.

  • Immune Function: Vitamin D helps support a healthy immune system by helping immune cells function as they should. It plays an important role in your body's ability to respond to everyday infections, including common seasonal illnesses.

  • Cardiovascular & Metabolic Health: Studies show low vitamin D levels are associated with early changes in blood vessel health and reduced insulin sensitivity in midlife women, though more research is needed to understand whether vitamin D is a direct cause.

How Do I Know If I’m Getting Enough Vitamin D?

Many women with Vitamin D deficiency don’t have any symptoms. That’s why it’s often called a “silent deficiency”. When symptoms do appear, they can include:

  • Persistent fatigue or low daily energy

  • Muscle weakness and slower recovery after workouts

  • Chronic bone or joint discomfort

  • Getting sick more frequently

The tricky thing is that these symptoms overlap heavily with other midlife problems, such as thyroid issues, iron deficiency, poor sleep, and the hormonal changes of perimenopause and menopause. That's why symptoms alone can't tell you whether your vitamin D is low. A blood test is the only way to know for sure.

Why Does Low Vitamin D Appear in Midlife?

  • Aging skin: As you get older, your skin naturally becomes less efficient at making vitamin D from sunlight.

  • Changes in body composition: Vitamin D is stored in body fat, so changes in body composition during midlife can affect how much is available for your body to use. 

  • Digestive health: Some digestive conditions, including celiac disease and Crohn's Disease, can make it harder for your body to absorb vitamin D. The same is true after some types of bariatric surgery.

  • Routine midlife testing: Many women first learn they have low vitamin D during routine blood work at a yearly physical or when they're evaluated for perimenopause or menopause. 

How to Know If Your Vitamin D Is Low

The only way to know if you're getting enough vitamin D is with a blood test. Ask your healthcare provider to check your 25-hydroxyvitamin D (25[OH]D) level. This is the blood test most doctors use to measure your vitamin D level.

Can Low Vitamin D Cause Hot Flashes? 

Vitamin D deficiency doesn't directly cause hot flashes or night sweats. However, studies have found that women with low vitamin D levels are more likely to report these symptoms.

Vitamin D receptors are found in the hypothalamus, the part of the brain that helps regulate body temperature. Researchers are studying whether low vitamin D levels may affect how the body responds to the hormonal changes that trigger hot flashes, but the evidence isn't strong enough yet to conclude that taking vitamin D directly improves these symptoms.

How Much Vitamin D Should Women Take? Understanding Dosage

The right amount of vitamin D depends on the current levels in your blood. That's why testing comes first; your results will help determine whether you simply need to maintain healthy levels or treat a deficiency.

If Your Levels Are Normal

Many women can maintain a healthy vitamin D level with a combination of food, safe sun exposure, and a supplement if needed. Good food sources include fatty fish like salmon and sardines, egg yolks, and fortified dairy products. Even with a healthy diet, though, it can be difficult to get enough vitamin D from food alone. That's why many women benefit from a supplement.

If You Have a Vitamin D Deficiency

If your blood test shows your vitamin D level is low, your healthcare provider may recommend a higher dose of vitamin D3 supplement for several weeks or months to help restore your levels. Once your levels improve, you'll typically switch to a lower maintenance dose. Your clinician will likely repeat your blood work to make sure your treatment is working.

Is More Vitamin D Better?

Not necessarily. You may see a wide range of dosing recommendations online, but more isn't always better. For most adults, 4,000 IU per day is considered the tolerable upper intake level unless you are being treated under active medical supervision. The goal isn't to take the highest dose possible; it's to safely reach and maintain your optimal target range without risking complications like elevated calcium levels.

How to Choose and Take a Vitamin D Supplement

1. Always Choose Vitamin D3 Over Vitamin D2

Vitamin D supplements come in two forms: vitamin D2 (ergocalciferol) and vitamin D3 (cholecalciferol). Vitamin D3 is the preferred form because it is identical to the form your body naturally manufactures from sunlight and is substantially more effective at raising and maintaining stable blood levels over time.

2. Pair with Vitamin K2 (MK-7)

Go for a vitamin D3 supplement that also includes vitamin K2. These nutrients work together to support bone health. Vitamin D helps your body absorb calcium, while vitamin K2 helps direct that calcium to your bones and teeth instead of soft tissues like your arteries.

If you're choosing a supplement with vitamin K2, look for the MK-7 (menaquinone-7) form, which stays active in the body longer than other forms.

3. Take Vitamin D With Food 

Vitamin D is a fat-soluble vitamin, which means your body absorbs it best when you take it with a meal that contains healthy fats. If you take it on an empty stomach, your body may not absorb as much.

If you’re choosing a supplement, go for one that combines vitamin D3 with vitamin K2 and omega-3 fatty acids. Vitamin D helps your body absorb calcium, vitamin K2 helps direct that calcium to your bones, and the healthy fats in omega-3s support vitamin D absorption. Combining these nutrients in one supplement can simplify your routine and help ensure you're getting all the recommended nutrients. 

Sunlight and Vitamin D

Sunlight helps your body make vitamin D naturally, but it isn't always a reliable source, especially during midlife. How much vitamin D your body makes depends on several factors, including your age, where you live, the time of year, your skin tone, and how much time you spend outdoors.

Because unprotected sun exposure increases the risk of skin cancer, you shouldn’t rely on it as your primary source of vitamin D. And, for many women, sunlight alone isn't enough to maintain healthy vitamin D levels. The best approach is a combination of safe sun exposure, vitamin D-rich foods, and supplements when blood work shows you need them.
